In addition to experiencing significant health disparities in multiple arenas, transgender and gender diverse patients face unique barriers to care. The purpose and rationale of this session is to present an educational model that has been designed to improve the health of transgender patients. The Community of Practice model engages experts from several disciplines and organizations to create a better curriculum for training health professions students that acknowledges the challenges that transgender patients face when trying to access health care resources and improves communication skills for working with transgender patients. 1. To report the findings of our work utilizing Communities of Practice at the National Center for Medical Education Development and Research 2. To provide model curricular elements developed for medical students and residents that can be used by health professions programs to prepare students to effectively provide gender affirming care to patients. 3. To initiate discussions about national standards in medical education for gender affirming care
