Designing Holistic Programs: Growth-Oriented Assessment, Feedback, and Learner Development
Description
PURPOSE:
Residency training requires learners to achieve competency while navigating career development, scholarship, professional identity formation, and wellness. We developed a longitudinal framework integrating mentorship, individualized learning plans (ILPs), competency-based assessment, and structured remediation within a four-year Obstetrics and Gynecology residency program.
METHODS:
The framework was implemented across 48 residents. Incoming interns complete a survey identifying preferred mentor characteristics to guide faculty mentor assignments. Mentors meet with residents regularly to review ILPs, support goal attainment, and provide guidance. Semiannual reviews integrate Clinical Competency Committee feedback with ILPs to assess milestone progression, career goals, scholarship, and development needs. Each Program Director follows the same resident cohort throughout training, providing longitudinal coaching and consistent expectations. Residents requiring additional support receive individualized Performance Improvement Plans informed by evaluations, CCC feedback, and prior ILPs.
RESULTS:
The framework established a continuous process for mentorship, feedback, assessment, and learner support throughout residency. After the first year of implementation, 100% of residents completed ILPs. Residents benefited from longitudinal guidance from both faculty mentors and Program Directors. The model has been well received by trainees, faculty, and residency leadership.
CONCLUSIONS:
Integrating mentorship, ILPs, competency-based assessment, wellness support, and remediation into a single framework promotes continuous learner development. A key innovation is the longitudinal assignment of a Program Director to each resident cohort for all four years of training, enabling developmental coaching, consistent milestone expectations, and individualized guidance that complements faculty mentorship.
