Description

Purpose
The Home Hospital (HH) model has emerged as a promising approach to delivering acute and complex care in patients' homes, improving patient satisfaction, and preventing hospital-acquired complications. Prior to initiation of our HH program at our institution, we implemented a novel Home Hospital Inter-professional Preparedness Simulation (HIPS) program. This abstract summarizes our implementation of our HH training program and feedback provided by participants and leadership.

Methods
Our HIPS program consisted of three phases: a teamwork exercise, assessment of a simulated emergency room patient to determine HH eligibility, and a home assessment of the patient and a scenario requiring escalation. After completion of the HIPS program, participants were asked to complete a survey assessing their ratings of each component of the program followed by the most valuable takeaways of the and sought feedback for ways to improve the program.

Results
Our HIPS program feedback was evaluated by all 19 participants where ratings ranged from 3 to 5 out of the scale of 5 for all categories. Specifically, the simulation of the ED Patient Interview, the 1st Home Visit, the Escalation of Care, and the Overall HIPS rating received scores of 4 or 5 only. The Maze Game Team Exercise and the Discharging HH Patient simulation each received a single score of 3, with the remainder being 4 or 5.

Conclusion
The HIPS program feedback data shows overall positive impressions from participants of an inter-professional preparedness program, with takeaways noting the value of the inter-professional exercises, ability to collaborate with- and learn from- other providers. This feedback show the value of simulation in the initiation of a new HH program and can potentially be expanded to train new staff. The inclusion of different professions highlights the value in training but also reflects the practice of HH. We believe that other HH programs would benefit from similar HIPS programs.
